SANDY, Utah — A grisly workplace accident left a Utah woman dead this week after she was pulled into an industrial bakery mixer.

Carmen “Jackie” Lindhardt, 45, was mixing dough at Ream’s grocery store bakery when she was pulled in and caught in the auger.

“Another employee was working, heard her scream, immediately ran over and shut that machine off, cutting all the power,” Sandy PD Sgt. Dean Carriger said.

But the machine didn’t power down right away and Lindhardt died in the bakery.

OSHA is taking over the investigation; Lindhardt had worked at the bakery for three years.
