WASHINGTON, DC -- Turns out, the Walmart truck driver who crashed into Tracy Morgan's limo had been awake for 28 hours at the time of the deadly accident -- a violation of New Jersey law.

The National Transportation Safety Board released its report Tuesday on the June 2014 crash that seriously injured Morgan and killed fellow comedian James McNair.

The report also shows that truck driver Kevin Roper could have prevented the accident had he slowed down.

Accidents like this are making the Federal Aviation Administration wake up to the dangers of on the job fatigue. A study shows that air traffic controllers' schedules often left them tired, less alert and at risk of endangering others.

Get this, 60% of controllers admitted to falling asleep on the job! And nearly 20% of them said they committed significant errors, like bringing the planes in too close together.

Scary stuff.

Maybe it's time to revisit those schedules before fatigue leads to something fatal.
